By attending a group sound bath or private sound therapy session with Sonora Wellness, I confirm that the information I provide is true and current. My information will be kept confidential in line with the Privacy Act 1988 (Cth) unless required by law or with my written consent.
I consent to receive sound therapy services and understand that participation is voluntary and at my own risk. I am responsible for my own health and safety during and after the session.
I understand that:
- Sound therapy is a complementary, non‑medical modality.
- Practitioners do not diagnose, treat, or prescribe for any condition.
- Any information shared is for general wellbeing only and is not medical advice.
- I will consult my medical practitioner before changing any medications or treatment plans.
I confirm that:
- I have disclosed all relevant medical, physical, neurological, or psychological conditions and will update the practitioner of any changes.
- I am responsible for seeking medical clearance if needed.
- I will inform the practitioner immediately if I experience discomfort or pain.
- Emotional responses may arise during or after a session, and I am responsible for seeking additional support if needed.
- Sessions are non‑sexual in nature.
- I may withdraw my consent at any time.
- Services are for clients 18+.
I acknowledge that:
- Sound therapy may involve holistic or reflective discussions, and I may accept or decline any recommendations.
- Participation may involve inherent risks (e.g., emotional release, dizziness, unexpected reactions). I voluntarily assume these risks.
- The practitioner is not liable for injury, loss, or damage except where liability cannot be excluded under Australian law.
- In group sessions, the practitioner is not responsible for the actions of other participants.
- Relevant information may be shared with other health practitioners only with my consent or where required by law.
This agreement is governed by the laws of New South Wales, Australia.
